The unique and varied circumstances of families seeking support from Brayden’s Gift will be taken into consideration by the Board of Directors. We will assess and award funding based on eligibility.
Application Checklist
In order to be considered for financial services, Brayden’s Gift requires the completed application and documents listed below. No application will be reviewed unless ALL of the following is submitted.
- Complete Application (no questions left blank)
- Autism Diagnosis with the name of Diagnostician/developmental pediatrician noted
- 2 Most recent tax returns
- Consent, Release, Waiver, and Indemnification Agreement
